It wasn't. The facet value doesn't have to be a topic; I misread the
DTD. It does, however, have to be an SGML NAME.

This is incorrect, since 0, 1, and 2 aren't valid NAME values.
See 5.5.2 in ISO 13250:2003.

If that were what an occurrence meant I would agree, but it's not. An
information resource is an occurrence of a topic if it is deemed to be
*relevant* to the topic. That's not the same.
